Hypertension Program

This is program will help you to set goals, remove barriers, and connect with community resources to better control your high blood pressure.

The Resource and Behavioral health teams will help you create a personalized plan to achieve your goals and provide support and accountability for 3 months.

Colorectal Cancer

  • Colorectal Cancer, or CRC, is cancer of the large intestine and/or rectum. 
  • It is the second deadliest type of cancer in the United States.
  • Talk to your provider about the best screening options for you.

Patient Experience

Waterfall Community Health Center considers it a privilege to be your healthcare provider, and we value your feedback. Every year, we ask for your input in a patient experience survey so we can learn more about our successes and where we need to make improvements.
