Patient Experience Information

Waterfall Community Health Center considers it a privilege to be your healthcare provider and we value your feedback. Every year, we ask for your input in a patient experience survey so we can learn more about our successes and where we need to make improvements.

Thank you to all our patients who filled out the survey! 

We are always looking to improve our quality of care; please see some of the patient feedback collected from the February 2023 survey. 

We are excited to share that most patients say their provider always shows respect for what they have to say and that the front desk staff always treats them with courtesy and respect.

It is our goal to continue to increase the respect that patients feel at each of Waterfall’s clinic locations.

We have also identified opportunities for growth/improvement in the following areas: 

  • Patient appointments for routine care.  
  • Same day responses to patient medical questions.

Waterfall is continually developing plans to improve quality of care. Current efforts include: 

  • Working with members of the Spanish speaking population to improve access to care.
  • Providing specialized care coordination for patients with diabetes and hypertension. 

Here’s what patients had to say about the care they receive at Waterfall:

“The mental health department at Waterfall is the best. 

The counseling services and the Veggie Hut have provided me with a great boost in my health and overall care. I feel like I am very cared for here. 

Starfish has been awesome in all aspects and has helped a lot! 

My provider has put me on the path for a healthy life, physically and mentally. As for the medical assistants and receptionists, they are always friendly and efficient.
