Waterfall Community Health Center in cooperation with Advanced Health, unveiled a mobile health unit in Spring of 2020. The Mobile Health Unit hosts our "Family Neighbor Team" and provides fully integrated healthcare services in a variety of roles for our community. Patient education, primary care, health screenings and social health efforts are just a few of the uses for this amazing resource! The Mobile Health Unit is a technological marvel, allowing both in person and remote access to complex medical analysis tools and provides a live, real-time uplink for providers back in our main clinic or even across the state when needed. Currently, the Mobile Health Unit is scheduled to be in Charleston, Oregon at the Bon Appetit Events Center on Boat Basin Road EVERY THIRD WEDNESDAY FROM 9AM-3PM.

The Mobile Health unit is scheduled to be in Lakeside, Oregon at the Lakeside Library on 8th Street EVERY FOURTH WEDNESDAY FROM 9AM-3PM.

Sports physicals are usually held in early august, please check back for next years sports physical dates. If you need one before then please connect with a provider at Waterfall.
