Patient Experience Information
Waterfall Community Health Center considers it a privilege to be your healthcare provider, and we value your feedback. Every year, we ask for your input in a patient experience survey so we can learn more about our successes and where we need to make improvements.

The October 2024 Patient Experience Survey (or CAHPS) showed improvements in the following areas:
89.7%
of patients report always or usually receiving a response to their questions within 72 hours.
88%
of patients felt their provider always or usually knew important information about their medical history.
76%
of patients report being asked about resource concerns (e.g. resources for food or transportation) at an appointment in the last 6 months.
Waterfall is continually developing plans to improve quality of care. Current efforts include:
- Working with members of the Spanish speaking population to improve access to care.
- Providing specialized care coordination for patients with diabetes and hypertension.
