Job Description
Job Purpose:

Under the supervision of the Chief Medical Officer, the  Nurse Practitioner functions in a team and mission-based setting to provide delivery of primary care to patients, including health promotion/maintenance, counseling and education, assessment, acute/chronic care, and consultation/collaboration with supervising physician, other health care professionals, and community resources. Exercises progressive levels of autonomy in day-to-day activities based on satisfactory job performance. Provides specialized nursing services in the area that may include school-based health care, women’s health care, family planning, and primary care; does related work as required.

Duties:

  • Adhere to the spirit of Waterfall Community Health Center to remove barriers to care and our mission statement while performing assigned duties.

  • Obtains and records client’s health history; performs comprehensive physical assessment (including biological, social, and psychological needs) as part of a total physical examination.
  • Demonstrate substantial knowledge of interviewing and counseling techniques.
  • Diagnoses, treats, and plans appropriate health care for clients in the area of school-based health care, women’s health care, family planning, and primary care as appropriate for specialty; refers clients to physicians and/or outside agencies as required.
  • Demonstrate skillful knowledge of nursing techniques and procedures, medical and lab equipment and testing procedures, medication.
  • Provides and prescribes medications within area of specialty.
  • Performs laboratory procedures and interprets lab test results.
  • Provides case management services for select clients.
  • Ability to provide group, individual and community education classes on birth control methods, sexually transmitted diseases, and other health concerns.
  • Establishes and maintains client medical charts and other necessary records.
  • Provides in-service education classes on assigned responsibilities for clinical staff.
  • Demonstrate skill in diagnostic procedures for diseases and the drug therapy needed for treatment. 
  • Proficient in the use of medical and laboratory equipment.
  • Participate in the Clinical Quality Improvement and Quality Management meetings and other meetings as requested by supervisor and/or Senior Leadership Team.
  • Demonstrate ability to work independently, communicate effectively both verbally and in writing, deal tactfully and patiently with clients, maintain effective working relationships with other related agencies, professionals, and citizen groups.
  • Promote individual and organizational integrity by exhibiting ethical behavior to maintain high standards
  • Maintain professional affiliations and enhance professional development to remain abreast with professional literature and current health care management trends and rules in FQCH regulation.
  • Performs any other duties needed to help drive our Vision, fulfill our Mission, and abide by our Organization’s Values.

Education and/or Experience and Qualifications:

  • Bachelor’s degree from an accredited school of nursing: Master’s degree in nursing; completion of a state acceptable Nurse Practitioner’s program; a minimum of one years’ experience is desirable. National certification in specialty is preferred.
  • Necessary special requirements:  Licensed by the State of Oregon as a Registered Nurse, certified by the State of Oregon as a Nurse Practitioner and Prescriptive privileges.
  • Language Skills: Able to communicate effectively in English, both verbally and in writing; additional languages preferred.
  • Ability to be credentialed with Medicaid, Medicare, private and other insurance.
  • Maintain confidentially – in accordance with WCHC policies and HIPAA regulations.

Licensure/Certification Requirements:

  • Licensed Nurse Practitioner (Oregon); Fully Credentialed and Insurable.
  • Current American Health Association (AHA) Provider Basic Life Support Card (BLS).

Physical Requirements:

  • Prolonged periods sitting at a desk working at a computer.
  • Able to lift up to 15 pounds occasionally.
  • Able to travel as needed.
  • Exposed to sick people.
