Astrid Diepenbroek R.D.H. Limited Access Provider, came to Waterfall CHC to promote and provide dental hygiene services after 20 years in private practice in 2008. "My first dental hygiene job was at the Open Door Health Center, in Arcata, California, in 1987. That was my first experience in providing integrated health care to a cultural, social and economically varied community."
In 1989, I moved to Coos Bay and was married to a self employed artist/potter. He died of cancer 5 years later, due to a lack affordable health insurance, preventive medical services, delays in diagnosis. After ward, I began to travel to Mexico, Domincan Republic, and Guatemala and serve many children and adults in obtaining dental hygiene and sealant services, realizing the need for dental health services worldwide. Watching the insurance industry change for the worst over the past twenty years, I felt a need to serve the vast majority of people who have no dental insurance "benefit", so as to maintain good conscience of whom I am truly serving - people, not profit driven organizations.
Obtaining the Limited Access Permit in 2002, is a great asset for many dental patients in Oregon, giving access and portability in facilitating dental hygiene for select groups that often have difficulty receiving care in dental offices. This permit allows dental hygienist to practice in a Community Health Center setting, with or without a dentists presence.
I have been a volunteer dental assistant and hygienist with Medical teams International Dental Van since 2003, working in Coos, Lake and Curry counties. Currently, I volunteer at the North Bend Health Dept. screening young children for early dental problems, placing cavity varnish and educating parents about good dental health practices. I have a part-time business, providing homebound and elder dental care, and have educated CNA's and public health nurses and WIC program providers about proper dental care and anticipatory guidance.
Yuriko Lee, Family Nurse Practitioner

Originally from Japan, Yuriko worked extensively as a Registered Nurse in Southeastern Arizona since 2001. Yuriko completed a Masters of Science in Nursing (MSN) at Emory University in Atlanta, Georgia, specializing as a Family Nurse Practitioner (FNP) and Certified Nurse Midwife (CNM). “My experience has taught me that good primary care along with a healthy diet and active lifestyle are all keys to maintaining health. I especially enjoy collaborating with doctors and Nurse Practitioners of like mind who also share their broad knowledge with their patients, giving them care and personal attention the way traditional family medicine was practiced with excellent results.

In 2002 Dr. Rajesh Ravuri moved here from Chicago. He completed his residency training at Rush Westlake Hospital and specialized in Internal Medicine. His special interests include Cardiovascular health, Diabetes including Insulin pumps, Gastroenterology including Endoscopies & Colonoscopies as well as Holistic care of the patient. To this end, he appreciates the Waterfall Clinic's mission of providing access to health care for all.
His practice motto is "Partnering with my patients in their pursuit of Health, Wellness and Happiness".

Alex Shade, Liscensed Clinical Social Worker (LSCW)

Alex Shade came to Coos Bay 13 years ago from Las Vegas, where she obtained her Masters in Social Work degree and worked in that community. She then began work with an adolescent residential treatment facility in Coos Bay which allowed her to do direct therapy with individuals, couples, families and groups. She then went on to obtain her Clinical Social Work License in the state of Oregon.

Shannon Thom, Family Nurse Practitioner

Shannon moved to Coos Bay area with her mother, husband, and two daughters in July 2009 from Northwest Washington State. She graduated with Associates Degree in Nursing from Everett CC in 1997, then received a Bachelor of Science in Nursing (RN) from Seattle Pacific University in 2003. She then pursued degrees in Family Nurse Practice (FNP) and a Masters of Science in Nursing from Gonzaga University in 2009. Shannon is board certified by American Academy of Nurse Practitioners in Family Practice.
Shannon has 12 years of RN experience in labor and delivery and family practice, as well as teaching nursing.
I am completely dedicated to providing holistic health care from the nursing model. I diagnose and treat most health conditions, but I have a greater focus on including the family, working collaboratively with patients, and including alternative and complimentary practices that nurture the whole person rather than just focusing on the illness or complaint. I readily consult with area physicians when I need guidance on more complex health issues.
Providing healthcare, and at a deeper level, healing, is both a profession and a spiritual calling. It carries a moral obligation to serve those who need it most.
